Physical Therapist –

Panhandle Home Health is seeking a full time or part-time PT to conduct physical therapy visits in patient’s homes. PHH serves Berkeley, Jefferson, and Morgan counties with the greatest need in Berkeley County. The qualified applicant may elect to be a full time or part-time employee with an hourly compensation or an independent contractor with PHH to provide the necessary services on a pay per visit compensation model.

The position is very flexible allowing for early or late start and stop times to your day and the ability to work remotely from home. Physical Therapists are a vital part of the overall care plan for patients receiving home health services following an acute care hospital stay for reasons such as post-op care, joint replacements, neurological disorders, etc. The focus of PT is certainly home safety and fall prevention in a patient’s very own home setting where it matters most!!

Health Information Services Clerk-

Panhandle Home Health, Inc. is looking for a full-time health information services clerk that will maintain medical records and perform insurance verification and authorization processes. This position is responsible for obtaining insurance eligibility, verifying benefits, as well as obtaining timely insurance authorizations.

Duties also include verifying the accuracy of visit notes and lab results, as well as physician orders and other duties as assigned. A qualified applicant will possess a High School Diploma or equivalency, education or experience in maintaining medical records, the ability to exercise sound judgment, and good interpersonal communication skills.

Registered Nurse –

PHH has a current opening for a full-time, 40 hour per week RN position. Our nurses go into a patient’s home to provide and direct one-on-one physician ordered skilled care to patients who reside in Berkeley, Jefferson, and Morgan counties. Potential for further education in wound certification exists.

Candidates must have a current WV RN license as well as current CPR certification. One year of experience is preferred but will consider LPN experience or less than one year of RN experience depending on acute care work history. The position is paid hourly and travel is also reimbursed. A typical day may be drawing labs, providing/teaching wound care, educating patients/families on disease processes. Home health is an ideal opportunity to use a wide variety of hands-on nursing skills. Regular hours are 8a-430p. Weekend, holiday, and on-call coverage is scheduled on a rotating basis and may average 3 days per month.

Quality Specialist –

PHH has made changes internally and needs to fill our last Quality Specialist position. This transition has been amazing and effective!!

The position is M-F 8a-430p with no more weekends!!!!

We are looking for a qualified individual with OASIS experience and Quality Assurance / Performance Improvement understanding with a desire to serve the patients and staff as we achieve the best patient outcomes! The applicant must be focused with attention to detail and able to multi-task in this fast-paced environment we call healthcare! This position promotes an organized approach to effective episode management, improved quality care, and streamlined communication among/throughout interdisciplinary teams!! It provides a point person for each patient that improves patient satisfaction while providing the applicant continued one on one patient contact although the position is not direct hands-on patient care.

Quality Specialist job responsibilities include:

  • Oversight of South Berkeley/Jefferson Co team and conducting weekly team meetings to ensure quality care, episode management, and coordination of services.
  • Fielding calls from patients/staff for patients in the assigned team.
  • Chart audits for patients in the assigned team focusing on OASIS and ICD-10 accuracy (training provided).
  • Annual supervisory visits with staff on the assigned team (this is a shared responsibility of all clinical leaders).
  • OASIS Certified – offered by Agency.
  • May be requested to take a supervisory call as needed
    Conduct Annual Fit Testing for N95 use.
  • Possibly serve as the Agency’s Wound Committee Chair – WCC is offered to all nursing staff as grant funding is available.
  • And of course – other duties as assigned.

LPN, Full-Time:

Panhandle Home Health is seeking an independent, organized, motivated and efficient LPN to fill a full-time Staff Nurse – LPN position.

The service area includes Berkeley, Morgan, and Jefferson counties. Responsibilities include providing direct skilled patient care in a patient’s home including but not limited to wound care, disease process education, lab draws, ostomy care and teaching, and foley catheter maintenance. The candidate would work directly with a supervising RN to assist with carrying out the patient’s plan of care as established by the RN and MD. Assistant Director of Clinical Services

Panhandle Home Health, Inc is seeking a qualified applicant who is dedicated, motivated, detailed, and efficient to join the leadership team at PHHI as Assistant Director of Clinical Services. The applicant must be a Registered Nurse with a current valid WV RN license and CPR certification.

This is a salary position with hours Monday – Friday 8a-430p and on average is responsible for supervisory call one week per month.

The ADCS is responsible for:

  • Infection Control Program.
  • Emergency Preparedness Plan.
  • Staff supervision providing direct patient care.
  • Occasional chart audits.
  • ICD-10 coding knowledge.
  • OASIS proficiency and review.
  • Serve on various committees such as Quality Assurance Performance Improvement.
  • Responsible for covering for the Director of Clinical Services during their absence.
  • Serves as direct supervisor to PHH intake department.
  • Maintains staff scheduling each month.
  • Oversees annual mandatory education assignments.
  • Prepares and monitors Continuing Education opportunities and credits for staff.
  • And other duties related to overseeing the clinical staff to ensure the efficiency of day-to-day operations of the Agency.
